click "new topic"
click the dropdown/spoiler thing for the member application format
copy format, paste into type window
fill in format
post

Then just wait for a Mod or higher to approve your application *-*

Note: it will ask for "vouches" but you can leave that blank if you want and people will just post their vouches in the thread.
